Depends what's on the form , you haven't told us !
Disclose any PCN numbers , name and address , email address , plus copies of 2 recent redacted utility bills as proof of I D under the GDPR law , also a copy of the V5C if you still have it !
No phone number
No driving licence
No passport
No photo I D whatsoever

If it has her name and address on it , then as an official government document it's one proof of I D !
The owner could be Barclays Bank or Toyota finance ! It's irrelevant who owns it ( ps , a V5C has never provided ownership details )

A V5C does not prove ownership and ownership is not relevant here , what is relevant is your partner proving that they are correct data subject before the data is released , due to data protection1 -
