Horizon Parking Fine

Hi, where is the actual template I can use for parking appeal?. I have been sent a PCN from Horizon Parking it says the entry time 17:19 and exit time £18:29. I did pay for the parking and the ticket was for 17:22 to 18:22. I was few minutes late as had 3 kids to sort out. Any help/ advice will be much appreciated.
