Do I still need to appeal with private parking firm to start that process?
    I would.
It's a simple case of sending the blue text template appeal easily found in the first post of the NEWBIES thread.
Send it unchanged - no additions or alterations needed.
    Send it as the keeper.

Ten minutes work. Why would you not do it?

Who actually cares what UKCPS have said?   I have no idea why you do!

Why are you calling it a fine?  This isn't a fine.

Why are you ask if this thing that you won't be paying 'will keep increasing'?

Why are you in such a tailspin, this is a template, BOG STANDARD appeal rejection letter.  Fully expected from this scam industry.  You know that the next stage is IAS because the silly template letter tells you that.  IT IS NOT AN INDEPENDENT APPEAL, IT IS RUN BY UKCPS' TRADE BODY.

However, we are finding that when people send tons of evidence (including uploading the proof of the complaints you made) PPCs are often not contesting IAS appeals at the moment, probably due to having staff on furlough and because the IAS stage costs them the princely sum of £15 (or £25 if they lose).  So calm down and do a pointless IAS appeal and see if that does the trick.

It is nor a fine.  If you have not already done so, complain to your MP.

The IAS will not let you appeal. They are an appeals service designed to support the scamsters that pay them.

Complain to the managing agent for the land in question , ideally you want a cancellation by the medical practice. Or the managing agent , or the landowner

So it didn't matter that I hadn't appealed within the 14 days?

What do I do now? Any advice would be very much appreciated :-(
As you likely were told at the time, you might have won. The IAS is allowing more appeals right now, presumably related to covid and staffing levels.

AS for what next - as ever, the newbies thread is the first place to ask.

My doctors surgery also got back in touch after 4 weeks of very little help, and said although they do not have the landowners details, they have the managing agents details?
Why the question mark?  It's obviously the MA that you want to complain to and who will have been dumb enough to contract with an ex-clamper firm with no appeals service with sniffing at, even if you'd been well.

Not sure why everyone always wants a next step that doesn't exist.

Clearly you ignore the PPC if the MA won't cancel it when you escalate the complaint.  And the NEWBIES thread tell you the only final stage where you need to take action, is if you get a LBC or claim form.  Which is when people win.
