Lastest advise from the BMA is for doctors not to get involved with this at all even when the patient offers to pay. Many GPs have told reception to not put such patients through at all.
If people feel they fall into one of the exemptions provided by the government they can self-certify that and presumably argue it out in court if a fine was issues.1 -

As do all staff in hospitals or healthcare settings even non-medical staff like me.

You could always wear a face mask exemption card lanyard. Apparently they are very easy to get hold of and you don't even have to prove you are actually exempt.0
