I don't remember what my form looked like. However it was the date, the time, and a brief summary of the incident and what had led up to it. Because I recall they added on that the sun was low, at my request.
I was then asked to sign it say I agreed it was an accurate record.
Whether that's a formal statement of the type you are referring to or not, to the uninitiated we're still signing a statement, whatever the plod call it.All shall be well, and all shall be well, and all manner of things shall be well.
