    That link you used is to request all the information DWP hold about you. To request just a copy of the UC85 form you need to contact UC as previously advised.

  • Just called uc, 4 min wait.. Funnily enough the person who answered the phone was my case manager..
    She said she will put the report in the mail tomorrow

    I believe when you ring it’s supposed to recognise your phone number if it's the one on your claim and direct you to your case manager.
    Information I post is for England unless otherwise stated. Some rules may be different in other parts of UK.
  • Ahh, I never knew that. Even before taking my name she said I am your case worker.. Then when she said her name it made a connection as she always replied to my journal message and the back pay was authorised by her.. I never knew they had that sort of system
