You need to have a frank discussion with your line manager.
Not to be awkward,however if you keel over tomorrow and are unfit to work.
You will be replaced in the blink of an eye.
so for your own health you need to be firm as to why you need to work your contracted hours.0 -

And just to make it clear i have asked my manager multiple times to change my hours and he has simply refused!
You stop asking and start telling.
As soon as rota is posted, send an email or memo (keep a copy) and tell him which 16?hours you will work and include reference to your medical condition and contract as agreed.
Then don’t turn up/ leave once hours are done.

I would be WRITING something similar to the text below. And sending it to my manager, their manager and HR. And if this did not produce a result I would be thinking about writing to the CEO. Oooo - and by this time I'd be thinking about selling the story to the papers.
Dear [manager's name]
I am writing to try to resolve the problem I am having with my scheduled hours.
I have a contract to work 16 hours a week. At my interview I explained very clearly that due to a serious medical condition I cannot work longer than this.
Despite this, I am regularly being put on the rota for a much larger number of hours. In the current week I have been scheduled to work 31 hours. I have made numerous attempts to explain to you that I cannot do this, but it has made no difference. Let me stress yet again that it has a serious effect on my health if I work too many hours - I am waiting for a heart transplant.
I am happy to abide by my contract and work 16 hours. Please confirm that you will provide a revised schedule of work by [insert regular date of schedule herel or you could simply say 'by Tuesday' if you think they will sort it this week]. In the absence of any further direction I will simply work for 16 hours as scheduled, and then go home. You will appreciate that given my medical condition I will, without any dishonesty on my part, be entitled to take the time after 16 hours as sick leave, and I will claim SSP following the appropriate rules.
I look forward to receiving your confirmation that my hours will be scheduled as contracted.
