First of all I want to know how to write a note on my credit report saying how I disagree with this fraudulent claim with me.

I thought this is the first thing that should be advised?

You can only escalate to the FOS or the ICO after the complaint process has been exhausted.

To go back to the questions you have not answered:
What is the 2010 debt for?
Is the account listed on your credit file?
What is the date of default marked on your credit file or what other markers are displayed?

Why are you asking if you know what to do then?

Contact all 3 of the CRA's and request this be done
