Looks OK to me, for a simple statement to accompany a consent to set aside a CCJ.
As for the other WS, like your own, it should be headed up Witness Statement, and look like this:

Should be fine. Main thing is that you have a signed consent order. Statements support how judgment by default happened.
Consider:
1. Did you correctly receive papers
2. Have you acted promptly to sort it out.
Lots of threads on the test applied. Hopefully it all goes through on the nod now as a paper exercise.0
