You can't help being ill and if it was a contagious illness, she would have become ill too, so would not have wanted to visit.
The train costs depends on if you can afford to pay and if she can afford to lose the money too?
Maybe a little discussion if you are able to do so?
I would definitely offer to pay if you can afford it. There again, she might be extremely rich and perfectly ok to lose the money - just because she's elderly doesn't necessarily mean she's poor! Some act like they are very poor but are in fact, very wealthy with a very high monthly income, so be aware!0
