I'm of the opinion its definitely best to ask for the PAC first. Don't have to be rude about it, and it's just a normal part of their business, but it saves everyone's time by not !!!!!footing around.

O "Hello, how can I help?"
C "Hi, I'd like to request my PAC please"
O "Certainly, I can do that for you. Are you thinking of leaving us?..."

Then you can go through the normal retentions stuff, if they can offer you a sweet enough deal to stay, then great, you can tell them not to bother with the PAC, if not they have to give you the code.
