Has there been a mistake (door hanging),can it be fixed?

Last week we had 4 internal doors installed as part of our house extension.The carpenter who installed them still has to come back and do other work around the house (and has been here before),but I am just a bit worried about the doors.
At the top of each door,there is a gap,as wide as the door,about 5cm (2inches) deep.
I have been given a few different suggestions as to what they may be: ventilation shafts,gap for (very narrow) window (!),mis-measuring of door,mis-measuring of door -space during construction,etc.
Can someone tell me,does it sound like there has been a mistake made,and if so,is it fixable,or is it deliberate?Getting a bit worried now.
Going to see if I can upload a photo to show,but if not,based on the description,is there a problem?

    Who bought the doors and who bought the frames?

    They're quite obviously mismatched.

    Really, very, not normal. Outrageously so.

    Was there an existing doorway there ?. If thats the case then the old door may have been a non standard hight so the carpenter has fitted a new lining head lower down so a standard door will then fit. He will possibly then fit wide architraves to cover the old existing lining head .

    If its a new lining and a new door then its clearly a balls up.

    I would add that as a Carpenter and Joiner I was always taught its good practice to fit 3 hinges on a door for various reasons that I could bore you with So I'm not impressed with the way the doors been fitted with only two hinges.

    Ive never seenn anty thing like that. !! What is the height of the door as it is now .
