Nexus 4 - USB Port Not Working For Data

As per subject. I have tried various cables and different computers but Windows will not recognise that there is a device connected, but the phone charges OK via USB. It used to work just fine - indeed I flashed the 5.1 update using Wugfresh NRT not so long ago - but now ... nada.
Any ideas? (I know I can use Airdroid etc. for transferring files, but that's just a workaround not a solution).
What made me notice this was initially wanting to copy a photo from my phone to my work PC (which also used to work OK). I just thought it was something had gone funny with my work PC. But when I tried my home PC it was the same issue.
Now I have the 5.1.1 OTA update that won't install because my phone is unlocked and rooted*. Without a USB data connection then it seems I'm scuppered .... unless anyone knows a way to flash the new 5.1.1 image locally on the device? (I have extracted the 5.1.1 tgz/tar package and copied the container folder to the SDCARD folder on my device).
Thanks for any advice ... if there's a way to do the 5.1.1 flash locally then step-by-step (idiot) instructions would be appreciated.

Some probably obvious points you've already covered...
Have you tried refreshing the drivers via the Wugfresh NRT?
Have you still got USB debugging turned on? Have you tried on and off?
Do you have an OTG cable you can test the port with using a pendrive? (not sure if root is needed for that though).
With mine I have to unlock the screen to see the actual data. It does still show the device anyway though.0 -
I dont think he needs an OTG cable just for the basic data connection and access to the DCIM folder etc.
Have you configured it for MTP or PTP - Try the other option.0 -
No he doesn't need an OTG cable at all - it's just another way to test the USB port.

Actually scrub the OTG option - you have to jump through several hoops to get it to work on the Nexus 4 which I'd forgotten about.0 -

There's one fatal flaw to that guide ... it still requires a USB data connection, as all the commands are typed on the computer and not the device.
Just reading further down the page - back in a bit.
Looks like no luck - I need root to try the local (non-USB) method and I don't have it any more (and don't know how to get it back without a USB connection - which brings me back to post #1).0 -
If initially all you want to do is install the update once you have it on your device can you not just flash it from recovery as it is an official update it won't matter what recovery you have on the device?
