    Diagnosis spot on. Question is how did it get that profile? If all the extensions are disconnected now will it screw up again when they are re-connected? OP needs to check they are all filtered if there is anything attached to them (Sky box, phone, burglar alarm, whatever).

    However, I think now IS the time to call TT and get that profile reset as suggested above, then see how it behaves....

    Burglar alarm ... these can be wired directly to the back of the NTE5A box. (They're not supposed to be, as only BT are supposed to touch the back, but that doesn't stop installers). If this is the case then there should be an inline filter (I can't remember the correct name) in the wiring from the back box to the alarm control box; if this isn't there then the alarm system can cause issues with the broadband and make the DLM system at the ISP think there are noise issues, so changing the profile.

    Sky boxes used to be particularly bad too - even when a filter was used. I don't know if it's still the case now, but old advice used to be to double-filter a Sky box connected to the phone line.

    bl00dy hell, I remember the first one of those I had, before it was common-knowledge about the double-filtering...sod of a fault.

    ..BUT, yes, there's soo many things internal & external to a property now that could cause severe noise...happy hunting OP :)
