Phoneboxdirect cashback claim form

Does anyone have a copy of the cashback claim form for phoneboxdirect that they could email to me (or post a link to)? Please PM me if you have.
I ordered a phone from them and they say it was in the email order confirmation, but I definitely didn't receive a confirmation email. They say they are going to post one to me, but I'd rather get one by email if possible, so if anyone has one that would be most useful

    make sure you get your order reference number that you should of got with the email as they want that on the cashback form

    Thanks, yes I got the order reference on the confirmation web page and in the shipping email.

    I usually puut both

    eg

    Total cashback: £330
    Cashback amount: £66 (£330/5)

    Not had any problems with it to date (had 22 successful claims so far)

    BEWARE: The address on the claim back form is INCORRECT. Get the correct address from their website in their terms and conditions.

    Are you sure? The only address I can see in the T&C is not for claims. The T&C say return the form to the address on the form

    To obtain your cash backs please send a copy of your bill to:
    MOBILE MEDIA SYSTEMS LTD
    Claims Dept.
    PO Box 8161
    Cotgrave
    Nottingham
    NG12 3WL "

    and state :

    "

    Claiming
    The claims address is on the cash back claim form we sent to you by email when you ordered your handset. Please return only one claim per envelope as we shred everything else we receive. "


    These are the instructions that I will be following for my new deal (and have followed successfully to get all 5 cashbacks on my last PBD deal) but yes there is a different (non cashback related?) address given at the very bottom of the terms & conditions page.

    Does anyone know of any reason to use this second address, not the one actually on the claim form ??
