How do I get medical evidence for esa

My husband has been on incapacity for nearly 10 years and received a letter this morning to go for a medical this Friday and to bring any medical reports with us but I don't know how we get medical reports. His neuropathy was diagnosed 10 years ago but they said theres nothing they can give him and it would just get worse so we don't actually see anyone about this. He went for a xray or something similar in january 2012 which showed he has damaged the main nerves in both hands and as this meant he couldn't push his manual wheelchair he was referred to have an electric wheelchair. I have ask at the hospital for a copy of this but they say they can't give it to us and we have to ask our gp for a copy but the gp says it hasn't been sent to him and we need to ask the hospital, we're just going round in circles. How does anyone get hold of medical evidence.

Mobilising unaided by another person with or without a walking stick, manual wheelchair or other aid if such aid can reasonably be used
Cannot either
a) mobilise more than 50 meters on level ground without stopping in order to avoid significant discomfort or exhaustion; or
b) repeatedly mobilise 50 meters within a reasonable timescale because of significant discomfort or exhaustion.
A prescribed powerchair is hard to get, and is not given on a whim.0 -
Don't you get copies? Every hospital visit I have they give me a letter (same one cc'd to gp). Every exray, CT scan, MRI etc i get a letter advising me of the results and what it means.
Other than that your GP records, social work records, carers schedule or other things.0 -
OP was the X-ray requsted by your GP? if so ask the receptionist if they will chase it up for you. If it was ordered by a consultant etc at the hospital you will need to speak to their secretary.0

If he cannot do a) or b) - then he is entitled to the support group.
A prescribed powerchair is hard to get, and is not given on a whim.

Really, whenever I've been for a scan or MRI I've never been had a letter from them, if they can give me the results there and then they will tell me but never give me a written version and to get one would cost me £20 or so, just shows how hospitals differ.0 -

The results of any scan I have are
A) communicated to me if NFA is neededin the follow up appt.
I received a letter today re my last MRI and it states no change since last scan 4 mos ago, consultant will review what this means at next appt.
I get copies after every hospital visit, scan etc.
They don't come from the hospital but from the consultant who ordered them.0 -

You need to speak to the specialist's secretary. A letter should of been sent to your GP and your husband. If you have trouble getting one from them ask the receptionist at the GP surgery to chase it up for you.0 -

It should be automatic - I get cc'd in every letter sent to my GP following every appt. even the "thank you for referring this lady to us, she is on waiting list and will be issued an appt in due course".0
