    The LBA-should it be sent by post and who should I address it to?
    Do I set a time frame to elicit a response before lodging the small causes complaint?

    Post 30 in my link above (post 4) covers LBAs. Send it to CPW (registered address on this dealer's website) Trading as Mobiles.co, by post but also send a copy by email to Mobiles.co - and, yes.
