Baby passport, my passport still in maiden name

I am applying for my baby first passport. My passport in still in my maiden name even though I am married. He has the family - fathers surname. I am really confused how to complete the application. Should I compete it as the mother with different surname on passport or does this not matter. Rang the helpline number and it is just automated line, no one to speak to.

Is the father named on the birth certificate? If so, everything will be clear to them. In any case, lots of women choose not to change their names so everything should be fine, but you might care to attach a covering note confirming the relationships. (Both of my sisters have kept their maiden name but have children who have taken the fathers' surname, and they have not had any trouble getting passports and travelling with their children.)0 -

Thanks, yes fathers name is on birth certificate but my name is married name on the certificate too not maiden name. Everything is in married name except my passport as i only got married 1 year after its issue, I kept in old name. Far too expensive to get a new one. Still got more than 5 years on it.0 -
Put your your married name @ section 4 and sign it in your married name. Put a note that your passport is still in your maiden name @ section 8. The child's birth certificate should show your married and maiden name on it if you were married at the time of birth so all the names will match up0

I have just had the exact same situation last month. Babies birth certificate is in my married name but passport is still in maiden name.
I spoke to someone at the passport office and this is what they told me to do -
In parents details section put your married name, ie name on babies birth cert along with your passport number.
In the extras notes section write a short note explaining you passport is still in maiden name.
I put a photocopy of our marriage cert in with the form for reference, used post office check and send and it was back within 13 days with no problems.

The passport people just called me back. They said put maiden name as is on passport but include marriage cert and short explanation. Do I not need to include original marriage cert. Do I need to send my passport as well.
I suppose the important bit is to include the explanation. We don't go on holiday till January so should be ok.0 -

They still need mothers details if father applies and proof of marriage don't they. I did think that at first but then it still needs my details as well.0 -
We have the same problem, i've been to the post office today, and paid to have the passport application checked.
Daughter also has passport in maiden name, so we just filled in the details, as it is on the passport, and all that was required was babys birth certificate, and the 2 photos.
the lady didn't ask for marriage certificate, but i did explain that daughters passport is still in maiden name, and she said that as long as everything is filled in with the correct details, it's fine.0 -

Thank you for that. I have just booked our holiday for January and don't want to have anything mess it up. I am waiting for the online form to be posted then I will get the post office to make sure it is ok. Thanks everyone for your replies.0 -
