If this is the first time the OP has missed an appt they may be sanctioned for two weeks. If the OP lies, the sanction could be far longer.

Yes, the letter will be returned to the DM and they will decide whether you will be sanctioned.
I really would be honest - if you keep lying and they find out the sanction will be far longer.0 - 
            I can completely understand how it's possible to forget, when I was signing on I would wake up in a panic first thing Friday (I signed on at 9am) trying to remember if that was the day I signed on or not, and since I usually got a lift with a neighbour so it would be organised the evening before it wasn't like I just rolled out of bed and signed on, the week was organised around it. In fact every time my neighbour couldn't remember whether I had gone the week before or not, so I was quite pleased it wasn't just me.
For the job interview - on here we can't advise that you lie, however if you did decide to continue down this route I would give them the details of either a friend or family member who could pretend they had an interview with you. They may only want email proof of it. They probably just need something to put in a box on your form.
However once I wanted to be late for signing on as I had a job interview, and the 2 weeks beforehand I sat there holding all the emails about the interview and they refused to let me be late and said my time wasn't movable which I later found out to be untrue, so it depends entirely on who you get.
Asking someone to pretend they are someone how they are not, in order to influence the situation is downright fraud.0 -

            I signed on 5 times between being on the dole and getting a job I was late for all but the first one nothing happened. Just ring up and say "sorry I forgot which friday it was because I have been feeling really depressed latley"
For being late say "sorry I overslept" if questioned say your starting to lose hope0 -

And they explain the "interview" as part of the blur do they? You seem to have missed the point that the OP has already lied to them.0
