Santander Cash ISA Transfer Form - unclear!

Hi thanks for reading.
On the Santander Cash ISA Transfer Form (I can't post links yet as I am a newbie but if you google "Santander ISA Transfer it's right at the top), part 2 (a) states:
"Do you want to close your current Cash ISA and transfer all the balance, including the accrued interest?
YES NO"
...and part 2 (b) states:
"Have you paid money into your Cash ISA in the current tax year? (A tax year is from 6 April to 5 April in the following year.)
YES NO"
Underneath it says...
"If you answered Yes to question (a), please ignore questions (c) and (d).
If you answered Yes to question (b), please complete questions (c) and (d)."
The trouble is, I (like many people surely) want to answer yes to both questions - so do I answer questions (c) and (d) or not???
It's quite misleading!

yeah, been designed to confuse the hell out of you!!! Perhaps just to ensure the Santander customer satisfaction index won't raise above 2 out of 10.
I answered yes to (a) and (b) and ignored the rest. On the grounds that 'yes' to (a) tells them I want them to transfer my total existing ISA funds, and since I won't give them my transfer form before the maturity date of my old ISA, (c) and (d) are irrelevant.Wouldn't know what to do if I had to give them the form before maturity of my old ISA, and they couldn't answer my questions about it on the phone, either.0 -
That's what I'll do then - so if there's a problem I can blame you

You're not the only one who was confused by the a-b-c-d questions.
It's been talked about on other threads on here.

I just hope you're not transferring from Halifax, because between Halifax & Santander, both mine & my gf's ISAs have been totally messed up.0

Where is says complete online - CASH ISA TRansfer Form - it just downloads a document saying your adobe isn't up to date but it is!
