Santander Cash ISA Transfer Form - unclear!

Hi thanks for reading.

On the Santander Cash ISA Transfer Form (I can't post links yet as I am a newbie but if you google "Santander ISA Transfer it's right at the top), part 2 (a) states:

"Do you want to close your current Cash ISA and transfer all the balance, including the accrued interest?

YES NO"

...and part 2 (b) states:

"Have you paid money into your Cash ISA in the current tax year? (A tax year is from 6 April to 5 April in the following year.)

YES NO"

Underneath it says...

"If you answered Yes to question (a), please ignore questions (c) and (d).
If you answered Yes to question (b), please complete questions (c) and (d)."

The trouble is, I (like many people surely) want to answer yes to both questions - so do I answer questions (c) and (d) or not???

It's quite misleading!
