You are missing the #PW at the beginning and the # at the end. Press the * key repeatedley to get the P and the W. You must enter it EXACTLY as it has been shown to you and the message.

Insert a WRONG sim card, one that doesn’t work in your phone. When it says phone restricted, press ‘undo’. It will ask you for a restriction code, and input the code into the box on the screen.
