not enough funds for it
unpaid fee applied
cheque goes to clear
now not enough funds as unpaid fees kick in
more fees for bounced cheque

You mean sequence of events. Not consequence.
So, in month 1, this happens. Why did you not complain that the bank hadnt cancelled the direct debit?
Then going forward over the next 4-5 years, how is it that they still hadnt managed to cancel them?
You get an unpaid fee applied and it costs you around £25. Painful but recoverable. Yet you said you were paying £200pm for these bounced SOs and DDs. So, why didnt you stop using the bank and cancel the payments?
I think you are asking a lot if you want us to believe that HSBC couldnt cancel standing orders and direct debits despite multiple requests over 4-5 years.

And I repeat for the xth time, it's only becomes a vicious circle if you don't deal with it at the start. It should all have been easily solved at the time if you'd complained and stuck to your guns.
If seems you didn't. Who's to blame for that?
Anyhow, there's no point continuing this as we're going round in circles and not advancing anywhere.
