i think so, just had a form thing at back of letter, but i took the whole letter with me showing entitlement.

so not sure what the form looks like the dla404 form or what ever. but one you get with letter

All the standard letters and forms have a code on them so that they can be easily identified. I don't have the same as you because the system has changed since my DLA404 was issued back in 2008! Hence the confusion earlier.

  • The entitlement letter and the 404 are one and the same thing. To help those who are confused the 404 precisely says :

    and the warning :

    and the form number :

    Hope this helps.

Thats the old style cert, the new ones are just normal letters and they are valid for 12 months only.

I am getting a Paravan tomorrow and it is already disabled classed, I have spoken to blackpool about my disabled tax several times in the last two weeks and things have changed so much recently and this cert is not current.
  • Ok sunnyone, that one was issued in early 2008

Yes to both, they hadnt put the attachment onto the email which is why I couldnt open it lol.

I changed the tax disc too and even though I havnt got the crash tested wheelchair yet I still have more freeedom because one of the big advantages a Grand Voyager Paravan has over other drive from wheelchair WAV's is that there are 4 wheelchair positions (and you can carry four wheelchairs and three adults if you want too) so my husband has taken the passenger seat out and my powerchair is their and I can just slide into it and drive out of the car, its the first time Ive been out alone in years!

    Thanks for asking.
