    Not a problem, ask away anytime.;)

    These questionnaires were recently issued by the financial ombudsman service (FOS) and I believe it replaced the first template letter on here, you keep a copy as well, in case you need to refer your case to the FOS later on, as it would help your complaint and the fos to enclose any copies of complaints you have sent to DFS, and if it goes that far, there is also a FOS complaints questionnaire as shown on the link below.

    So not to confuse the FOS questionnaire with the FOS complaints form, (where this actual one is used to make a complaint to the FOS, if your not happy with the outcome);)

    If you want to write a cover letter, that is optional, the thing with using template letters for reclaiming is - as you know they all looked the same to the companies complaining to, and your own letter with your own wording is better chance of success.

    You can of course write a cover letter if you want to, if you feel you've more to add, for instance if you need to give further information.

    Hi we also had a suite from DFS was over 4 year interest free. ours was also with hitachi capital. we wrote to D.F.S stating we were mis-sold ppi and we never asked for it. We received a letter back offering to re-write the agreement without the ppi which we accepted they actually refunded all money paid and because ours was at the end of the agreement we got all the money back then the new agreement was wrote up over 6months so i just paid it off. this left us with the refunded ppi. one of the easiest i have done. Good luck!

  • I have sent an email to them requesting a copy of the agreement but will follow up with a letter i think.

    So DFS have come back to me today and have said that they have traced our order but are no longer able to supply a copy of the invioce. They can confirm my order details in writing however.....

    Does this sound right? Or are they just trying to avoid sending me anything which would have proof of the PPI on there?
    I guess i will have to formally write to them with my request now that going through customer services hasn't worked.

    You can still make a reclaim though, because they will still hold the main of the info from the last 6 years and details of the settlement should be included.
    So if you want to request a Subject access request (SAR), this is what you send to request the above info, there is a template letter for this, and if you do this enclose £10 cheque or postal order and they have 40 calendar days to comply.

    In their email reply to me, they have provided our order number with them - they simply say they are no longer able to supply a copy of the invoice.

    Given that i now have the order number with DFS and the agreement number with Hitachi Capital Finance, do i need anything further before sending the completed FOS Questionnaire to them?

    From reading the questionnaire, i have to provide the policy number of the PPI i am claiming about (DFS order no) and then later on the aco!!!! number the PPI was to cover me on (Hitachi agreement no)?

    With SAR's they vary, but so far with mine reclaims they have been of some help, they are supposed to send details they hold on your going back as far as 6 years at least.

    When we have made reclaims the loan number was that of the insurance number - because the insurance added within the loan.
    So I always put down that account number.

    Whatever account numbers you have then I would write them in and in brackets write (account number for Hitachi) and (account number for DFS), so they have a clear picture of the account.

    You do not have to enclose anything with the questionnaire, unless you wanted to write a cover letter, this is optional, and if you have further information to give.:)
