No, Orange do this as well, new SIM card on switching from contract to payg

Yes thats correct. You can't use a pay & go sim for contract and vice versa, they have different kinds of Serial numbers for each set.

    Well, I did.

O2 said you can, and issued and used the PAC to process the number from an O2 contract to an existing O2 Pay and Go SIM.

How many times does it need saying?

You don't need to, but you can. There; both are correct. :)

It definitely seems that whatever you do within deals with phone companies it is advisable not to take anything for granted and check as many details as you can think of.

Friend of mine switching from T-Mobile PAYG to contract, T-Mobile said they cannot transfer his number, he must port out to Asda etc and port in again.

That is ridiculous, if your friend went into a TMobile store and asked them to transfer his PAYG number onto a contract they should instantly be able to do this on their system. TMobile customer services should be able to do the same.

If a customer ports out with the intention of porting back in, the number needs to be on the other network for one month before it can be ported back into the original network.

Some networks are virtual (like Adsa or Tesco) and use another 'real' network to carry their traffic.
Your definition does not explain the difference between a service provider and a network operator.

    Example
O2 CPW is not a network operator
O2 CPW is not a MVNO (mobile virtual network operator)
O2 CPW is a service provider
