I took the pruhealth offer up on 19th September. The first payment came out on 28th. Transaction showing £0 on Quidco.

I signed up on the 26th of September using the link from Martin's article.
The offer ran out on 27th November.
I called Pru today to activate my free membership and the man advised me that I was not eligible for the gym offer but the £100 Boots voucher. Told him that I specifically signed up because of the free health club membership which was running till the day after I signed up.
He cannot do anything. He has passed the matter to the Vitality team who will come back to me within 48 hours. I hope they can give me the gym membership otherwise I will cancel the policy. I will let you know how this goes.
My policy document comes with a pack which details the things you can do using this policy. Under the Holmes Place offer, it states that the Holmes Place membership is under a 12 month contract so I don't know how you can cancel after 6 months. Your initial contract may be with Pru, but I think you are signing a separate agreement with Holmes Place as well, so you may be liable to honour the full 12 months.
For me that still works out cheaper as the full membership is £62, and with my Pru premium and £25 for the gym, i'm still paying under £50 for a very plush gym!
UPDATE: I got a call back from Pru. They have given me the 6 months membership which activates in two days! All I need to do is go down to the gym with membership number and PruHealth card/ID. Was very impressed with their customer service people. Very polite, extremely helpful.
I was also told it's a 12 month contract but at the end of 6, they give a grace period where the customer can decide to carry on or stop. If you do it within 30 days of end of 6 months, you don't pay activation/admin fee of £25, and membership is £25 a month thereafter.
Considering the membership is normally £62, even with my premium of £23, it works out cheaper. Nice one Mr.MartinMoneysavingExpert :-)0 -

Mine too - apparently it's not a problem.
From the quidco blog (http://www.quidco.com/blog/?p=32):
