At the fresh claim stage there is no requirement to do anything other than write down an answer (Where 'Yes' has been ticked) and potentially provide a payslip or other piece of evidence.
People seem to pretend that filling this form in is an onerous task when the reality is it is pretty straightforward. The people who dont even fill the 'Personal Details' section are beyond me - fair enough if they are illiterate but the rest are just lazy.0 -
I have not seen an ESA form but was surprised when I saw a DLA form as people give the impression it is a pig to fill in because it is so long - well like the IB50 (for Incapacity Benefit), most of it is just tick boxes with room to explain further if necessary.
My husband filled in last IB50 in in a couple of hours.
I too think unless you are illiterate or have learning disabilities or a problem with understanding things as in Aspergers' Syndrome there should be no problem filling it in. It is long, yes, but not complicated.(AKA HRH_MUngo)

update: still no news that i have passed or failed!? i have hit a stumbling block with my GP's receptionists though needed a med cert on 5th this month called at 11pm that day to book appointment told to ring back 8 am in the morning! couldnt get through next day was engaged so went down to book it after i took my kids to school, only to be told that i was to go home and ring 8am next morning !!!!!!! it was only ten past nine! asked why i couldnt book an appointment at desk and reply was we only book in for med certs at 8am till 9 am! via phone practice policy i'm affraid asked to see the practice manager low and behold she was on holiday! and the gp was busy (to be fair its a small gp practice and he's see's allot of people on daily basis) so again phoned engaged and the next day phoned engaged and the next untill got through ten to nine next available appointment was 15th! had to take it, told receptionist that note was due 5th and would it be possible of a back date NO i was told, was fking fumeing with her, asked to speak to practice manager, who was still on holiday! (nice for some isnt it) so got my sick note and it states thate note was due 5th march 2010 but had 15th date on it so i'm hopeing that ill get my regular money.
i know many would say its down to you to book appointment for the day its due, with family problems and my forgetfullness and due to depression i find it hard to track dates i have a diary but forget to check it or missplace it may sound like an excuse but its genuine.0 -

Yep, you should have made absolutely sure you had an appt and the cert. in plenty of time - there are no excuses and getting angry with the receptionist is utterly pointless and unfair.0 -

to be clear my GP's receptionists are what i would call ignorant! (you can stand at the desk they will talk amongst themselfs knowing you are waiting the phone will ring look at you and say wait a second and answer the phone then after answering the phone and when GP buzzes for the next patiant will leave the desk and go into gps room to get forms signed, will then come back to the desk to either answer another call or look up notes on the system before they tend to me this happend today and was waiting at desk just to ask if prescription was ready for my 5 yr old for over 10 mins, i have been refused prescritions by the receptionist when they are due not by the GP himself, they have lost repeat prescriptions i have submitted, they dont pre-book appointments for med certs and advise you ring the day before or on the day of the cert runs out between 8am and 9am as to not block any emegency calls for start of surgery at 9am, they wont back date so they claim but then again they kind of did by puuting due 5th march 2010 on it in brackets, you cannot get any straight answers from them one will tell you yes the other say they dont do it!! you just cannot speak to the prac manager as she is never there or on holidays, and when you speak to the GP he just says you cant get the staff these days, i had an incedent with a receptionist, i went in to see doc in the morning he sent me for an x-ray i was due sick note next day so asked my note is due tomorrow but because of the incedentrequiring the x-ray he couldnt give a defo on the amount of time i would need off from work told me to have x-ray and phone down with the results and he'd tell how long i'd need off, i did this and told his receptionist i needed to talk to him they put me through i told hime wich he confirmed as he'd deen the report on the system told me i need x amount of time off and to tell the receptionist when im put back through that i was to have the sick note for x amount of time, was put back through to receptionist told her needed the note from today as was separate to normal note as per results she blatantly told me NO i was to ring down next day as doctor needed to assess me for a note i explained that i had already been assesed if she looks on system at my appointment and she still said no i have to be on site and be assesed by my gp i explained that gp has already said i am to have a note as he has asessed me and that the call was to inform him of the results so he could know how long i would need to recover i also explained that i had requested the form originaly on site, she then told me no you cant have one you have to be assesed and to make an appointment or she could book me in as a favour for three weeks later!! i had to call the LHB to sort things out and the receptionist still saw fit to call me at home from the surgery and verbally abuse me! because i had contacted the LHB and made a complaint against her, i was in the right i could request a cert over the phone as i originally requested on site and had been assesed by the GP and confirmed over the phone and he checked the system on the report, a receptionist over ruled her boss! and deny my rights to a legal document. thats the kind of surgery i deal with on a regular basis.
My GP is wounderfull i have no complaints against him it was not his fault that happened and he apologised to me for his receptionists foolish mistake and her attitude towards me and unprofessionalism of how she dealt with the complaint to wich i told he he need not apoligise for his receptionists mistake she need to apologise to me! wich she did face to face and a written letter. needless to say that my GP had to file a full report to the lhb what actions he took against her, wich was dismissal (she was on her probationary period) am i proud she was sacked NO we all need to earn a living do i blame her in anyway for her mistake NO why because she did not have the adequite training and understanding of how a GP's practace is run due to the manager not being there to properly supervise and train her, has there been any improvements you may ask yourself answer to that is NO as theres been so many practace manager in the last 12 months all implementing certain policy's of their own and from others that the receptionisits are somewhat confused on what policy to follow.
as for recently allot of things has happened in my family that my head is overloaded and i genuinly forgot about the note being up, as the GP receptionists do not allow pre booked appointments and have an hour to call each morning to book in for that morning and with one landline whats the chances of getting through that day? almost none the landline do not accept bt call back so cannot press 5 to request ring back. the surgery 's receptionists are a shambles and quite often rude and ignorant play god with peoples prescriptions and decide whether or not you can have them that decision is ultimatly down to the GP not them. i or they would have no complaints if they operated like they did a few years ago with med certs 12-1:30pm monday-friday you could walk in the practice after morning surgery give them your name and wait your turn to have your note. yes you may have to wait but after 1:30 they would stop the bookings and would back date a note to atleast 3 days prior under certain circumstances. just went down hill when in house practace manager left and numerouse area managers came in. shame really as the GP's name is getting tarnished because of receptionists.

(spelling police caution to not to bother nit picking and go back down to the money saver arms because i just cant be bothered with you and i do not give a HOOT). (takes a really deep inhaltion of ventolin breathes out sighs and thinks WOW I THINK I BETTER SEE THE DOC FOR STRONGER ANTIDEPRESSANTS!!!!!) i say in advance that if i seem harsh as to having a go at viktory i am not honest you have to forgive me that i do have blow outs or vents due to my mental health, its not me talking its the voice in my head!!!!! (dont worry im not that loopy..........or am i? ill let you all decide that im keeping that one close to my chest)0 -
I have to ring my Dr at 8am too. So you start ringing at 7.55 and carry on without a break until you get through. Never had a problem, have always got through just after 8. . Why leave it until 8.50?(AKA HRH_MUngo)

trouble with my docs everyone knows to ring well before 8am and keep ringing so is almost a lottery whether you get through or not, i take my kids to school for 9 am sometimes 8:30 for breakfast club as they love to hang with friends before class starts, so doesnt leave me much time to get hold of docs. i suffer with depression and dates times and stuff just get lost in my head and with family worries ATM we are all occupied on one thing so small thing tend to get forgotten till someone says something to jog the memory.0 -
