Right, there doesn't seem to be much about this particular branch of Caudwell Holdings, so I thought I'd post my current findings about them and also a question at the end. I've written once or twice before on this excellent forum -this is an update!
Got a 12 month contract on o2 200 with an SE K750i last October with £150 cashback.
Been absolutely fine so far, until month 6, cashback time!
Sent off the original 6th month bill (as requested) made copies of it, for myself and filled in and then printed off the form, as emailed to me once I'd requested it.
Sent the first one through via recorded delivery to the PO Box address as stated on their website. What a mistake! It was never delivered and Royal Mail has said it's lost. I can claim back 12 first class stamps however (whoo!) and I've been asked to send to RM, the original reciept.
After informing phones4u of this, they said no problem -send us your 7th bill. To be safe, I sent an original of the 7th, a copy of the 6th and a letter detailing what had happened via special delivery. Phones4u also said that as it had been lost, it won't matter that it might be late. I asked for email confirmation of this, my address was taken, but I've not received any confirmation.
Tracked it on the website, it was sent Friday, and it was delivered, after a redirect on the Monday (15th May). Phoned them up today, now the 18th, and they said that they've no record as of yet, as it might take up to 10 days for the claim to be registered.
It's skirting a little close to the deadline but should it be okay? It has now been delivered but, assuming nothing happens in the next 10 days, going over the deadline, what action can I take then? Any advice anyone? Cheers in advance.

I'm afraid that unlike wickleashley I have sent in my claim form for cashback from phones4u about a week past the deadline. What are my chances do you think? Does anyone have any advice for pursuading them that they really do want to pay me? Or even a contact number for them?!! Their website isn't very helpful at all! Thanks xxx0
