List as many as you can think of who you're registered with. They won't check up on you, and the bigger the list the more impressive it looks. They're looking for experienced MS'ers, so if you presently work for other companies, let them (Tern) know.

No, you don't need to register again. I was already registered for self employed before I started MS'ing and didn't inform them. I just include it on my tax return separately and haven't had a problem on last two tax returns, so would have thought they'd have queried it by now.
