get the dine in for two thing from M&S (something that can be kept warm if LO needs fed) and a nice DVD, change the bedsheets without being asked (I know that's not a hubby job in our house, but clean sheets are so lovely) and organise a nice bath for her with candles and bubble bath, fluffy towels etc..
Do it just after baby is fed and you can take him for winding/settling/whatever, to give her an hour for herself. Make it clear this ISN'T foreplayand that it's just because she is working so hard you wanted to do something to show your appreciation (note to any other new dads - it doesn't have to be your OH's birthday to do this little lot!!)

My first thought was that you could get one of those cute little baby outfits that says something like 'My mom's the best' or 'I have a yummy mummy' etc etc.
It'll make her smile and will look adorable on your new baby.

Also - how about a charm bracelet or other piece of jewellery incorporating your baby and wifes birth stones
I had a charm bracelet and for special occasions hubby or friends would contribute ie 1st baby he bought me the silver stork carrying the babe, friend bought the silver champagne in the bucket, etc I look at it now, (sworth a fortune, its full!) and every charm holds a memory....don't actually wear it much now, have become more of a "pandora" girl, same thing really!!If you're not sure where you're going, you'll probably end up somewhere else.......:D0 -
My partner bought me a bike. No don't laugh - it was the best thing he could have done. It helped me get back into trim quickly and later on we fitted a baby seat and all went cycling together.
Exercise, fresh air and no fuel costs!! All free - brilliant!!
