Any cheap way to use mobile minutes for international calls?

Have a look at 18185.co.uk.
I think you can dial a UK 020 number (you pay 4p connection fee per call) and then it's 6p per minute to Pakistan or 1p/2p per minute to US (landline/mobile). In addition you pay for the cost of your mobile call, but I think you would be using your inclusive minutes for this.
I haven't used this exact service, but I use 18185 for my landline calls to Switzerland and have not had any problems (and have saved a fortune!)

Better still for some destinations you could open an account with http://www.localphone.com/ who will allocate you local UK Geographical Numbers that point direct at your final destination numbers (no callthrough) and they dont charge any connection charge.
Pakistan only 5.2 p a minute .0 -
I got a Lebara pay-as-you-talk SIM and I am happy with the rates. If you top-up £20 or more until 5 January 2009 you get instantly 50% extra free credit. I asked some Lebara promotion guys and they told me that Lebara is using the Orange network. Therefore, you can expect the quality to be similar to the Orange one. I haven't experience any problems at all and I have been saving lots of ££ comparing to my Vodafone mobile which is extremely more expensive. I have to carry another handset with me though, but I think it's worth the effort.

Just as a postscript - both Lebara and Lyca run on the back of ORANGE so I have also not had a any problems in 2 months useage.Better to light a candle than to curse the darkness. :beer:0

There are several such brands - Globalcell, IDT, Lebara, Lyca, Mobile World from CPW, Nomi, or using Yourcallworld on a T-mobile SIM - but the OP is asking about using about using inclusive minutes on existing contract SIM cards, which would be slightly cheaper.
There have been callthrough providers emerge from time to time which use mobile prefixes for int'l calls, but the main networks have worked fairly hard to identify them and will exclude them from inclusive minutes or bar them.
Look on the MSE Callchecker for account-based callthrough services using ordinary landline access. And several of the Betamax VoIP brands have free calls to USA up to a fair use limit.

you can use service like topupdiscount (or topupnow /teletop). You top up by texting something like 'call on' to a premium number (costs £5.00). Suggest you cancel by texting 'call stop' or something similar straightway otherwise it keeps topping it up automatically. The rates are quite cheap.0

I have been using teletop to call a US mobile from UK landline for some months now and on the whole it's proved fantastic.. I do have one query though - quite often I connect to the access number and dial the destination - being greeted by a 0p per minute message.. when i get this I know I don't get through to the mobile so I have to redial - it should be charged at 0.5p per minute This redial costs me another 5p connection fee.,. no great shakes but recently this happens for 50% of my calls. Therefore half my credit is wasted. I have all inclusive calls via Virgin to the access number so in general im happy.. very happy....
