Aye, there are 3 options available on the website...
1) PDF - 58 pages - print it out and manually complete.
2) PDF - 43 pages - an 'active' PDF that can be completed on PC before printing out.
3) On-line - Via Siebel eService... the one I keep having problem with !!! There is a PDF version than can be generated, but it does say "customer copy - not for posting" ...0 -

Obviously I cant speak for all doctors and all conditions, but most conditions have symptoms that doctors can report on even if a diagnosis is not yet established... We get a number of reports with doctors writing no diagnosis yet in the medical condition box.
Like I said though, some doctors are great, others seem to think their lives would be a lot easier if us sick people didnt keep bothering them!
Thanks, maybe me and my mum both just have bad luck of the draw with doctors.
My mum's GP is still refusing to write any kind of letter until she has a diagnosis.
My GP and me have fell out badly over this and I am currently waiting to get into a new GP surgery.0 -

I currently receive higher rate care and low rate mobility for my son who is 13 (he has ADHD and Autism along with bowel problems) it is currently under review for renewal in March 09. alongside that my husband who is the Navy has just had a draft to the Netherlands and we are all moving there. After trawling the website am I correct in thinking that as it is an EU country I can still calim the Care component but not the mobilty component.
I would prefer to stay in this country but have some problems myself and am finding more difficult to look after my son by myself as he is getting so much bigger and I am only wee. I am in need of his father to help with his care. Which was one of the considerations the Navy gave in this draft.
I have tried myself to claim DLA as I have nerve damage to the base of spine (confirmed by physio after approx 10 years of treatment) who wants me to see a lumber specialist my leg goes numb and gives way (hence 3 sprained ankles and 1 break in the past year)
I have also been recently diagnosed with chronic irritable bowel syndrome (I have bought a key from Radar due to urgency and not always making it!!) I also suffer with monthly Migraines, high blood pressure, high cholestorol and throw in a bit of depression (I know putting me down would be more cost effective) Can I try claim DLA based on these medical conditions and can I claim it given I receive DLA for my son and I will be living in the Netherlands, my GP will obviously be forwarding my records to the Netherlands but he is willing for me to try and claim again, but I don't want to waste every one's time if I am not likely to get it0 -

Firstly, thank you for putting yourself on this site - it's an admirable action.
My query is about what I should do next. I've been refused DLA at the first application for my son who is 1 and who is severely allergic to dairy, wheat, egg and potato. I want to know whether to send a letter in giving lots more detail and extra letters from Consultant and GP to back these up or to go straight to appeal.
If it's best that I send a letter asking them to reconsider the decision can you give me some tips about how exactly to design the information please.
