Aaaaaaaaaw ((((((((((daisy)))))))))) don't worry about the screengrab, you've got the code and you emailed the immediately (possibly even within the 10 minutes) so I'm sure they'll be able to see that you genuinely had problems submitting the form.

I had no problem filling the form, in fact I used RoboForm to complete it. I use IE hth xxx
