Bottom line In our office's opinion you cannot withhold bonus proof.
It's not superseding it though, in fact the link I posted is a document giving guidance on the Code contained in the link you posted (I'm not sure that something dated March 1996 can supersede something dated February 1999 in any case!).
The only relevant part of that document is where it states:
"The intermediary shall not withhold from the policyholder any written evidence or documentation relating to the contract of insurance."
As it is worded 'relating to the contract of insurance' I take this to mean documents pertaining to the substance of cover provided (policy booklets, certificates) rather than proof of bonus.

I recall this coming into force before I left a major uk car insurer and we were advised against withholding ncb. I have discussed it with my colleagues who have many many years of experience and knowledge and they are of the opinion it can't be withheld but as I said clearly each co. will have their own take on it.0 -

But in any case the Code is obsolete since the FSA regime was implemented in 2005, and nothing in the FSA's ICOBS prevents withholding documents if monies are outstanding.0 -
I would pay the old insurer immediately, get the NCD "proof" to stop the cancellation of the new policy within a week, and then ask for a full breakdown and resolution.
Arguing about the old payments isn't going to solve the immediate issue, and may create additional problems if your new policy is voided....0 -

Pay the money to resolve the immediate situation, then pursue your complaint in writing.
As far as charging you for the time you hadn't provided the NCD, it sounds to me that the other policy wasn't cancelled and that's why it took so long to get the evidence. In this case, you aren't entitled to the discount as it was being used elsewhere.
If the company prior to Swiftcover cancelled from the same time as SCs policy started - well you might have a case to argue that the NCD should be applied from the inception date0
