Transferring a Nationwide UT ISA

I have a Nationwide UT ISA which is now managed by Legal & General and I want to transfer it to HL. Does anyone know if I can transfer it by just completing the transfer form or do I have to speak to the Nationwide FA who set it up in the first place? I'd like to transfer it as funds rather than cash because I want to keep one of the funds and later sell the others within the HL ISA for other better funds. Also I have been paying by DD into this every month and the Nationwide FA has kept some of that money outside the ISA with the intention of putting it in this tax year because I paid too much in ISA terms last year. It annoys me too because I'm never sure just how much has been invested in the ISA this year so far because they've stopped sending the monthly contract notes. I feel I've no control over my own money and can't invest in any other funds because I'm not sure exactly how much has already been invested this year, though I can guess. Anyway, I've stopped the DD cos I'm fed up with it, but what's the next best step to give me back control of my own money?
