Who to send ISA forms to ? (Birmingham Midshires ISA)

I get really worried when companies supposed to be looking after my money dont seem to know what they're doing!
I have a maturing Birmingham Midshires (BM) ISA which I will transfer into their 1 year 6.15% fix - no problem.
I also want to transfer previous years money to the same product with BM .... two things...
First:
I have just spoken to someone at BM who said send the transfer form to THEM, ... However... at the top of the form it clearly says...'Send this form directly to your current ISA provider. Do not send to Birmingham Midshires as this will delay the transfer of your funds'
Has anyone done a transfer with BM - who do I believe, lady on the phone or the form?
Secondly:
She also said that thay would open the ISA with my maturing funds and then add the transferred funds later - maturity date of the ISA would be anniversary of the first deposit.
Do all providers work differently? Nationwide will only open an ISA when they get the funds from other provider ... and last year ABBEY did it by opening a variable ISA to start with, waiting for all the funds to arrive from different providers THEN transferring to fixed rate.
Doing it BM's way I will lose out on a (small) bit of interest as the transfer in will only attract the interest for ten to eleven months - also it will be difficult to check the interest calculation on maturity ....?!?!?
Sorry for such a long post and grateful for any comments - anyone got experience of transferring in to BM?
