Transfer Natwest --> A&L

I've successfully opened my A&L ISA Account, and made a 'test' deposit from my linked account which worked fine, so I intend to fill up the remainder (£3,600 this year yeah?!)
That's all good, however, I now need to transfer my Natwest ISAs to the A&L Account, now they sent me a form in the post, this contains:
Secondly, there is a part in the form towards the bottom:
All good, but what evidence of investment can I enclose?! I dont have a 'passbook' either? - What, if anything do I need to send?
That's all good, however, I now need to transfer my Natwest ISAs to the A&L Account, now they sent me a form in the post, this contains:
- National Insurance number
- A&L ISA Account number
- Natwest ISA Account number
- Natwest branch address
- My signature
Secondly, there is a part in the form towards the bottom:
'My Evidence of Investment / Passbook is enclosed. The funds will be transferred to a Cash ISA Which I have already opened with A&L'
All good, but what evidence of investment can I enclose?! I dont have a 'passbook' either? - What, if anything do I need to send?

As far as the safety of posting is concerned, even if you hand the form into an A & L branch, they will post it to their ISA centre....;)0 -
I suspected they would just post it themselves anyhow, so I may as well do it myself.
Yeah, There's no mention of the passbook thing on the other form is there - I printed that out ages ago when you linked me to it before Baldur, perhaps Ill give them a call and see what they have to say!0
