Boiler only part working

I have a Baxi Combi Instant gas boiler. It provides both hot water for taps and radiators. The cental heating part works fine; so does the supply to the shower. But, hot water to other taps is very unreliable. Sometimes the hot tap just runs cold for ages - after 3 or 4 minutes I give up. If I turn the tap on againg a minute later hot water comes almost immediately, but only lasts for about 30 seconds.
The boiler fires up when a hot tap is opened - so where is the hot water?!
Can anyone give me a clue as to what is going wrong please. Replies gratefully received.
