water rates query,meter vs fixed bill

Hello can anyone tell me how water companies (welsh water)apply a fixed rate,is it decided by your average use in the past or a fixed price decided on house size or area ?
my reason for asking my aunties water bill has risen to 20 pound per week due to her son developing a nervous condition(ocd) whereby he has to wash all the time.shes struggling to pay and the water company are getting really heavy with her,threatening letters ect.i've recently discovered you can opt for a fixed rate,could this be a cheaper option ? anyone have any advice
