One Touch Switch Error 1114 – Unable to switch from Utility Warehouse to Vodafone or Sky

I'm hoping someone can help as I've run out of ideas.

I'm trying to switch my mother-in-law's broadband from Utility Warehouse to Vodafone using the MSE broadband deal (£115 voucher).

Every attempt fails with:

Error 1114 – Your details don't match your account.

I've also tried switching to Sky and get exactly the same error.

However, BT recognises the line correctly and appears happy to proceed with the order.

I've checked and rechecked everything:

• Correct Utility Warehouse account number.
• Correct service/landline number.
• Correct surname (exactly as on the bill).
• Correct address.
• Tried both the account number and service number in every logical combination.
• Tried Vodafone via MSE and directly on Vodafone's website.
• Vodafone Live Chat also couldn't complete the switch.
• Utility Warehouse say everything is set up correctly on their end.

Some history in case it's relevant:

• Originally Shell Broadband
• Migrated to TalkTalk when Shell sold them the broadband business.
• Then transferred to Utility Warehouse a few months ago when TalkTalk sold the business to UW.

The broadband is Openreach FTTC (up to 76 Mbps).

At this point it feels like an industry One Touch Switch/TOTSCo matching issue rather than incorrect details.

Vodafone can't complete the transfer even when placing the order themselves via Live Chat, which makes me think it's more than just an online form issue.

Has anyone seen Error 1114 before, particularly with Utility Warehouse, or have any suggestions for what to try next?
