The cashback insurance offers are a lot better than 1%, I got £49 back off a £350 policy. OP talked about Amex not being accepted so is having to find different (probably more expensive) policies just to earn a few points worth a couple of quid unless you can stack up loads and find a suitable flight to use them on - it's a viable comparison. 60,000 points for £300 in Nectar - would they get 10,000 points from buying insurance on the card and factoring in the policy is more expensive if the cheaper one doesn't take AmEx…

But was it a fixed £49 or was it a percentage? Obviously makes a difference if your premium is £3,500 rather than £350.
I dont shop around looking for somewhere to take AmEx but dont recall any cases of them not but then again I avoid the bottom feeders and buy on value rather than price; many dont take this approach though.
Generally I agree cash back is likely to be more valuable but its not mutually exclusive so you can get your £49 cash back and your 350 MR points (or 3,500 MR points if your insurance is more expensive)
There can naturally be exceptions though, if you are 350 MR points away from enough points to book a £10,000 first class ticket for pennies (I know it won't actually be pennies) then maybe the extra spend is worth it because of the time issue

The TopCashBack insurance one isn't stated as a straight %, it's currently set at £45 regardless which is what I got last year and the year before

Not done this myself but have you looked at the BA e-store? The comparison sites are on there and perhaps one or two insurance companies (guessing by their names).
I am guessing you will get Avios from the comparison site if you first go through the BA e-store then choose a policy from said comparison site, whether they accept Amex or not.

You can also use a cashback site with a insurance comparison site to get cashback.
It's good to be aware that going direct doesn't always pay - some insurers offer specific deals via comparison sites not available direct or via cashback websites.
