How to fill in this form? (JP Morgan)

Hi,

I want to transfer matured Sharesave shares out of JP Morgan Workplace solutions.

They have the following online form to fill in.

Are they asking for my name and address or the name/address of the bank/platform I want to transfer to? I contacted them a week ago via the online query form but still haven't received a response. I need to get this done within 90 days of me buying the shares and preferably before end of tax year.

To which stockbroker are you trying to transfer them to? It's that stockbroker's details you need. Usually you can ask your new stockbroker to initiate the transfer and deal with the details.

This is not a normal ISA transfer. It's a matured Sharesave scheme that I want to transfer into an ISA with a different provided.

The only way to transfer out of the holding account with JP Morgan is by completing the online form. They don't allow any other ways. The receiving bank can't start the transfer, I already asked.

If it was normal S&S ISA to S&S ISA transfer, it would be easier but these shares are not in a normal account, they're in a holding account (so I have to give them instruction where to transfer it to).

Okay, but where are you sending them? If to e.g., AJ Bell you can ask AJ Bell for the details to fill in that form plus your account reference.
