    That definitely looks like waterlogging, does it squelch if you walk on it?

    As suggested, try the foul water for leaks, overflow. It would only need to seep over long period, not a flood.

    The other is, as suggested, a lack of drainage to start with when you prepped the area

    Yes, it's squelchy, no evidence of water leaks though. The ground was prepared to within an inch of its life prior to turfing, and it was never an issue over the past thirty years.

    Have you lifted the inspection lid and checked it runs freely?

    Do you know if there is soak away under the lawn as maybe full with all the rain that have had.

    You will need a set of keys to lift the lid where the block paving is and it will be heavy.

    Have contacted the firm that installed the lawn?

    Definitely looks like there has been a flood there or water standing either on top or under.
    I had the fire brigade turn a burst hydrant onto my garden. Not fun to come back to.
    The lawn looked like this as do playing fields and the like after heavy rain can't drain away enough.
    Probably a result of exceptional dry summer and then a lot of rain.

    But I'd check the flow of water from the drain too. 
    If you can, or can get someone else to lift the lid (old ones are lighter than newer) though the only real way to tell is someone with a flexible pole with light and camera on the end and that's a bit costly.
    Ask on the DIY forum about checking and if there's a way to DIY. Give them a photo and link to this thread.
