    Nationwide FlexPlus travel insurance seems to be the best (only?) option. Thanks, particularly to friolento and TadleyBaggie.  I expected the need to declare (and pay for) pre-existing medical conditions, which we both have. 
    There have been recent stories of people with ongoing medical conditions being refused renewal this year, despite having been long term Flexplus customers.

    The suggestion being that Aviva took everyone on at changeover of insurer, but are now being more selective about whom they accept, hopefully you'll be okay.
