Surely this isn’t right. Has anyone experienced something similar? Any advice would be hugely appreciated

You don't ring these firms or set up payment plans. You also need to read the official MSE Guide to Parking tickets as that's set out in easy bitesize chunks.
These PCNs are (arguably fraudulent) 'double dip' ANPR error charges and you DO NOT pay these firms for unfair or plainly wrong PCNs.
Get onto your bank and report it as a scam - be VERY CLEAR with that word - and stop the payment plan DD. This is as bad a scam as push payments. You took a LBC too seriously then. Never phone these people.
Do not use a template to respond now. Your case needs certain things said.
Or just use the short one for LBC stage that you read about in the official MSE Guide, adding to it that this is a 'double dip' ANPR error case and DCB Legal must back off immediately and return the file to the client, to check for the 'orphan images' they missed. And that authority to take, request or receive payments from your account 'the payment plan' is immediately revoked.

Whose name is on the form - yours or hers?
There is no danger of a CCJ if you follow the process outlined there and detailed here. DCB Legal will likely discontinue when the claim is defended (at a later stage) and you'll pay £0.
