That's a little disappointing for us, given you are meant to be following the 8 steps which tell you all about that form (every question)!

It is also the simplest of forms. Don't put off the easy paperwork or let yourself be overwhelmed. We are with you. This is the only tiny little form that you need to fill in during this daft process.
