Then you are putting yourself in an extremely vulnerable position. You need to go to the doctors, ask for a note and make sure your employer is aware of the reason. The only person losing out by keeping your health problems quiet is yourself.
I have a relative in a very similar situation, major health issues and depression/anxiety. Cant face going into work. He has seen a doctor, had therapy on the NHS within 2 months, joined 2 self help/men's groups and has been off work for 7 months now. His employer is being very good about it.
Help is outthere but you have to ask for it. Your doctor should give you a test when you visit, the results of which will determine the help you need. They will also give you phone numbers to call should you need instant help.2 -

My Doctor can't change my meds. It has to be done by the psychiatrist and I can't get an appointment0

There has to be a bar otherwise everyone would say they were sick rather than going to work. If you don’t disclose it, they can’t distinguish you from those who are playing the system and would assume that is what you are trying to do.
In short, until you have another job lined up, you can’t lose your job for a reason that DWP would consider to be your fault (and they might well consider not taking performance management as that in the absence of medical evidence.) If you have a sick note then you can’t be at fault.
