Sorry to hear that, make the meds a priority. The system is tough to navigate once your off their radar.

Why, what's the point going sick? I would have to disclose my condition which I'm not willing to do
If anything it will make them back off as they will have to take the Disability Act into consideration

They won't back off, they'll just use it against me. They're dentists. They make terrible employers.

Yes, my employer doesn't know my condition. I have hidden it for 3 years...

Then you are putting yourself in an extremely vulnerable position. You need to go to the doctors, ask for a note and make sure your employer is aware of the reason. The only person losing out by keeping your health problems quiet is yourself.
I have a relative in a very similar situation, major health issues and depression/anxiety. Cant face going into work. He has seen a doctor, had therapy on the NHS within 2 months, joined 2 self help/men's groups and has been off work for 7 months now. His employer is being very good about it.
Help is outthere but you have to ask for it. Your doctor should give you a test when you visit, the results of which will determine the help you need. They will also give you phone numbers to call should you need instant help.

Yes, I need a meds change as my antidepressant is no longer working but I'm still waiting for an appointment with the mental health team. I haven't bothered them for 2 years now but there is no help when I need it.
Your doctor can change your meds. You have 2 threads running at the same time, there is advice on the other thread.

My Doctor can't change my meds. It has to be done by the psychiatrist and I can't get an appointment

If you choose to leave, you would have your UC sanctioned (no money coming in for at least three months from a quick Google.) Clearly when you have no savings, that isn’t an option. So you need to be be dismissed (not being good enough at the job would be fine, if there were no mention made by the employer of the performance management offer) or go sick, so that either your employer would still be paying you or there would be no grounds for DWP to withold benefits from you.

There has to be a bar otherwise everyone would say they were sick rather than going to work. If you don’t disclose it, they can’t distinguish you from those who are playing the system and would assume that is what you are trying to do.

In short, until you have another job lined up, you can’t lose your job for a reason that DWP would consider to be your fault (and they might well consider not taking performance management as that in the absence of medical evidence.) If you have a sick note then you can’t be at fault.
