I have similar and ongoing problems with the hach key/star button on my iphone - admittedly there are not many iphones around! Sounds like another civil service linked failure when NS&I claim - Horizon like - that its the consumer thats at fault. Inquisitive journalist urgently required!!

This journalist did some stories on the chaos at NS&I back in 2020.
Might help someone else... before I updated my Firefox browser this time I thought I'd try adding another trusted device - so I opened up my Chrome browser and tried to login. As expected NS&I prompted me to add a "new trusted device" and gave me a call to tell me the code to enter on my pc...
When prompted to listen to the code, I duly pushed the star button and then I got the silent treatment, so I tried a couple more times - carefully pushed the star button, listened, nothing... just about to hang up and I thought... it keeps looping and asking me to push the star button again - let's try quickly pushing the star button multiple times... and hey presto - it worked!
Probably pushed the * key about 8 times before it replied - I missed it the first time but did the same again and eventually caught the rest of the code
