Refund agreed for parking charge but no refund made

in 2020 I received a parking charge from a hospital carpark. 
I argued due to it being unfair and the parking manager at the time refused to cancel the charge. 
I paid the fine but sent a regular complaint by email stating my dissatisfaction at the unjustness I felt.

The emails went ignored until September this year when I got a response to say that the parking manager no longer worked there and they have looked back and agree that it was unfair and will process a refund to my original card. 

I responded to explain that the card used 4 years ago had since expired and asked them to confirm how they would prefer to make a refund. 

I have contacted them now 5 times since September and they have ignored me. 

I cant even remember what card I would have used 4  years ago to pay to look back. 

What should be my next steps baring in mind its £60 and wouldnt be worth incurring additional costs at this stage.

    Who made the refund offer. ?  You have provided very little information so far

    The persons job title on the email signature is: 'Group Car Parking Manager'
    They are employed by the NHS Trust who owns the carpark.

    Complain about it to the CEO at the trust, in writing, Via PALS or HR if necessary, plus complain about it to your local MP and ask them to complain about it to the trust on your behalf 

    Emails should be sufficient, with proof of your identity such as a copy of the V5c or 2 recent redacted utility bills and a copy of the paper trail so far, plus your account details for payment
