Claiming DCA on behalf of someone who has passed away

Hoping for a bit of advice here. 

A few months ago I saw Martin talking about these DCA agreements. My wife over the years has had two different finance agreements with two different providers - one being with MotoNovo and the other Blackhorse. 

My wife sadly passed away earlier this year, and had she been here, she would have tried to claim these herself - so I decided to try. I am both the sole beneficiary and executor of her estate.
 
I used the reclaim tool on the MSE website to generate the email to send to both finance companies, which I altered accordingly to state all of her details along with the finance agreement number, as well as saying that I was acting on her behalf and included my details and said that I can provide proof of me having authority to deal with her estate when requested. 

I sent these on the 13/09/24. On the 11/10/24 I received a reply from MotoNovo asking me to complete a further details form on their website. In the email it says:

"We have been unable to verify your identity based on the information you provided so far. To make sure that we can complete a thorough check, please could you fill out this form with as many of the details as possible as they would appear in your agreement".

When I go to the form on the website it asks for the agreement number, vehicle registration, agreement length and monthly repayments. I can give them the agreement number and vehicle reg, but have no clue about the other two. 
Then, the bit that is really stumping me... it asks for "your details", so name, date of birth, address, telephone number etc. 

So the question is, how do I fill out the "your details" section? 
Do I fill out with her details, even though it's obviously not her filling out the form, as those would be the details linked to the finance, and then if it comes back that there was a DCA I can sort it out from there and provide them with my details and proof of me handling her affairs? (though I would have to put my email and contact number in as she actually deleted the email account linked to that finance before she passed away, and her phone is rarely turned on these days). 
Or, do I fill it in with my name, date of birth etc. as it is "my details", even though these details would not match the finance agreement? 
Sadly there's no box where you can freely write at the end of the from for any additional information!

I did fill out a contact form on their website the other day to ask this question, though I've heard nothing back from them! 
It's so much easier to deal with a human rather than these online forms!!

    As you are the executor of the estate, have you made them aware that you wish to register your DCA issue (note: there is no claim, nobody knows what, if any, money will be paid) in this role and you are not doing it for yourself?

    If your wife had taken out the finance you should include her details with the agreement but be prepared to contact them, perhaps by email if they provide one later, and make note you are acting as the executor so there is no confusion.
